Registration Fee:
1. SH, EA4HP, and CSHP members ($350) (including conference material, 4 breaks, 2 continental breakfasts, 2 lunches, 1 Welcome Reception, and 1 Gala Dinner)
2. non-members ($400) (including conference material, 4 breaks, 2 continental breakfasts, 2 lunches, 1 Welcome Reception, and 1 Gala Dinner)
3. Number of family members/ friends at the Reception ($25 each)
4. Number of family members/ friends at the Gala Dinner ($50 each)
5. Number of family members/ friends at the Reception and the Gala Dinner($75 each)
Registration deadline is on February 20th, 2024. Registration is set at a maximum of 500 attendees, and a waiting list will be generated afterwards. It will be processed on a first-come-first-serve basis, and the case submitter has priority. A complete registration requires payment in-full in accepted forms as indicated below, and a confirmation e-mail from the Local Organizing Committee.You need to check your spam folder if you do not see a confirmation email immediately.

Terms, Conditions, and Cancellations:
1.To qualify for the member discount, your CSHP, SH, and EA4HP membership status must be current.
2.Registration is only considered complete and valid after receipt of confirmation e-mail from the Local Organizing Committee of the Joint CSHP-EA4HP-SH Workshop 2024.
3.Payment needs to be made in US Dollars, net of any bank charges.
4.Cancellation can be made before April 1st, 2024 , and a handling fee of $50 will be deducted from the refund; NO cancellation can be made after April 1st, 2024 .
